               Liability  Policy
By this release, I (we) (named above) are releasing all liability claims
against Little Haleos, llc or Jerry and Jennifer Hale, home owners due to
the act of pet boarding.  This release effects any obligations or claims by
either party. All dogs are boarded and cared for by Little Haleos, or any
of it's staff without liability for loss or damage from disease, death,
running away (escape), transportation, theft, or fire. During our initial
interview, it is your responsibility to assess our facility and decide if
Little Haleos is the right boarding place for your dog(s) and if you are
comfortable with the risks that come with boarding with us.

I understand that I am solely financially responsible for any damage or
harm caused by my dog(s) while under the care of Little Haleos. I agree
that in admitting my dog(s) to Little Haleos, they have relied on my
representation that my dog(s) is/are in good health, have not harmed or
shown aggression towards any person or any other dog and have not
consistently escaped from areas of confinement.  If my dog(s) becomes
injured or ill, Little Haleos shall have the right to call a veterinarian of
their choice; or to administer medicine or give other advisable
attention, within their discretion and judgment, and such expenses,
being reasonable in amount, shall be paid promptly by me, the owner of
the dog.  

If my dog cannot safely adapt to a group boarding environment, Little
Haleos shall have the right to place my dog in a traditional kennel of
their choice, or in my alternate boarding choice as stated above.

I have read and understand the terms set forth above.  I agree to abide
by all the terms, conditions and statements of this boarding contract.  
By filling in my name and the date below, I am agreeing with Litte
Haleos  liability policy and I want my dog to be a dog and accept all the
risks that come with that choice.
At Little Haleos, we do our best to ensure the safety of the dogs we look after.  During
your dog's stay, they will have contact (unless otherwise agreed upon) with other
dogs.  Due to the unpredictable nature of animals, you are leaving your pet with us at
your own risk.

                 Just in Case...
At Little Haleos,  we have a responsibility to our clients to ensure the safety of their
loved ones.  To make sure that our environment is as safe as possible we have a
non-aggressive dog policy.  If your dog should become aggressive, they will be boarded
at our local kennel or the kennel of your choice as stated above in the form.
Fleas and Ticks

All dogs must be treated for Fleas & Ticks. (Natural remedies are acceptable) If you do not treat
your dogs for fleas and ticks, we will administer a Capstar tablet when they get here and then treat
them with Front line at the rate of $3 per Capstar and $15 per Frontline.

We do not allow flea collars & all flea drops must be administered at least 5 days prior to
boarding at Little Haleos, please respect this because dogs will be playing with each other and these
chemicals can get into another dog's eyes or make them sick.
